   <subfield code="a">The surgical experience can be a very daunting time for the patient. The nurse plays an important part in helping the patient face this experience with minimal difficulties. Anxiety is a natural reaction to circumstances that are unpredictable and threatening. Thus to patients who will undergo a surgical experience, it is somehow an expected response of patients to be anxious. There are numerous ways that the nurse can use to help the patient reduce stress. One of which is Guided Imagery. &#13;
The study aimed to determine the effectiveness of Guided Imagery as an adjunct to structured preoperative health teachings on decreasing the anxiety response of preoperative adult patients. The participants are either male or female, ages 21-65 years old, about to undergo a major, elective surgical operation, able to understand, write and read Tagalog, and would undergo a major elective surgical operation for the first time.&#13;
The study utilized the quasi-experimental design. There were 32 participants in the study; 16 participants in the control group who received preoperative health teaching alone and 16 participants in the experimental group who received guided imagery, coupled with preoperative health teaching. The anxiety responses were measured in terms of physiologic indicators and individual scores from Zung’s Self-Rating Anxiety Scale. An open questionnaire was also utilized to provide descriptive experiences of the participants who underwent Guided Imagery. Different of two means and general linear model was used to measure the quantitative data and a thematic analysis was used to examine the responses from the qualitative data.&#13;
A mean difference of 0.112 for temperature, 0.580 for Diastolic BP, 0.544 for Systolic BP, 0.724 for Heart rate and 0.067 for respiratory rate was observed in the control group. For the experimental group, a mean difference of 0.168 for temperature, 0.013 for Diastolic BP, 0.002 for Systolic BP, 0.544 for Heart rate and 0.030 for temperature. For the control group all physiologic indicators are not significant. For the diastolic blood pressure showed a significant mean difference. Based on the General Linear Model, Within-Subjects effect (Factor, with or without Treatment), physiologic indicators showed significant difference except for the Heart Rate, given by Temperature (0.034), Systolic BP (0.014), Diastolic BP (0.023), Heart Rate (0.708) and Respiratory Rate (0.004). With regards to Within-Subjects effect (Factor*Treatment), values showed no significant difference in all physiologic indicators, given by, Temperature (0.815), Systolic BP (0.128), Diastolic BP (0.121), Heart Rate (0.708), and Respiratory Rate (0.872). For the General Linear Model, Between-Subject Effect (Control vs. Experimental) all physiologic indicators showed no significant difference. Statistical analysis showed that there is no sufficient evidence to suggest that the decrease in the physiologic anxiety indicators cannot be attributed to the Guided Imagery intervention (Treatment).&#13;
The Zung’s Self-Rating Anxiety Scale showed that for the control group, 6 participants who have normal anxiety levels in the pretest retained a normal anxiety level, 4 participants who have mild to moderate anxiety levels in the pretest retained a mild to moderate anxiety level, and 1 participant who have severe anxiety level in the pretest retained a severe anxiety level. There were 5 participants who have mild to moderate anxiety levels in the pretest that changed to normal anxiety level in the posttest. For the experimental group, there were 5 participants who have mild to moderate anxiety level in the pretest retained a mild to moderate anxiety level. There were 2 participants who have mild to moderate anxiety level in the pretest that changed to normal anxiety level in the posttest. There were 6 participants who have severe anxiety level in the pretest that changed to mild to moderate anxiety level in the posttest. There was 1 participant who has severe anxiety level in the pretest that changed to normal anxiety level in the posttest. There were 2 participants who have normal anxiety level in the pretest that changed to mild to moderate anxiety level in the posttest.&#13;
The participants from the experimental group verbalized feelings relief, relaxation, tranquility and lightness all of which are positive responses during and after the Guided Imagery intervention. The participants also believed that Guided Imagery really provided good effects for them. Positive effects include relief of stress, tension, fear anxiety and pain; lightness; tranquility,; and promoted positive thinking and mindset.</subfield>
