<subfield code="a">Introduction: The use of the IMCI strategy in the management of Danger Signs and Cough in
children ages 0-5 years is being currently implemented in the municipality of San Juan,
Batangas using the Inter-Disciplinary Approach (IDA) of the University of the Philippines —
Manila. Comprehensive training programs on the IMCI module for Barangay Health Workers
(BHW) have been started as part of the five-year plan of the Community Health Development Program. In line with this, the study aimed to evaluate the level of the competency of the BHWs in three barangays in San Juan, Batangas in skills performance of the IMCI Danger Signs and Cough.

Methodology: The selection of barangays included in the study was done using convenience
sampling. Target population of the study included all the 37 barangay health workers of
Calubcub 2.0, Nagsaulay and Subukin, where the researchers were assignedl. Twenty-
seven BHWs participated in the study.
Demographic data of the participants were obtained, and a tool which followed protocol on IMCI dangers signs and cough was adapted and utilized during observation to evaluate the skills performance of the participants. Focus group discussions were conducted after each observation to assess the participants’ thoughts, feelings, difficulties and constraints.
A scoring system was developed in order to quantify the data gathered and to facilitate data gathering, i.e., to determine the competency of the BHWs in skills performance based on the 80% set passing rate with a perfect score of 74 points, and to determine the relationship of the personal factors (age, educational level, and length of service) to the skills performance of the participants.

Results:
On Skills Performance:

The scores of the BHWs in the skills performance were computed and described. Only ten BHWs were able to meet the 80 percent passing rate of 59 points.

On Difficulties and Constraints

Majority of the participants from all barangays identified IMCI Cough assessment as
the hardest among the three competencies in performing the IMCI Protocol. On Thoughts and Feelings
Common responses included feelings of nervousness and tension due to having someone directly observing and checking if what they were doing was right or not. Perceptions included feelings of incompetence and not having adequate knowledge and skills to assess a sick child properly.
Some of the participants felt that there was a need to continuously train and review to further develop their skills in implementing the IMCI modules as well as move on to the other modules in the said material. However, a small number of participants stated that they find the training given to them difficult. When assessed further, some of the enumerated responses included inappropriate skill testing since it was done using a laptop.
Suggestions given by the participants included actual training on children to be able to perform the skills they have learned in theory.

On Personal Factors and Skill Performance

A weak inverse relationship (-0.16, a=0.429) between age and the skills performance
of BHWs was yielded. This suggests that as a BHW gets older, they are becoming less
effective in performing the skills in IMC] Danger Signs and Cough.
A weak direct relationship (0.11, a=0.561) between length of service of the BHWs
and their skills performance was obtained. This finding suggests that as the BHWs have
more years of service, they are becoming more effective in performing the skills in IMCI
Danger Signs and Cough.
A very weak direct relationship (0.06, a=0.744) between BHW/s’ educational
attainment measured in years and their skills performance was yielded; however,
relationship is almost negligible.
But since the level of significance was large for all parameters, the correlation that
was established may be attributed to coincidence.
Conclusion: The findings of the study showed that the only minority of the BHWs showed
competence in Skills Performance in assessing, classifying and managing IMCI Danger
Signs and Cough.

The personal factors of the BHWs including age, educational attainment, and their
length of service have no significant relationship to skills performance.

Recommendations: There are three major recommendations of this study. First is the
application of this evaluation tool to all BHVWs who underwent the IMCI training for IMCI
Cough in San Juan, Batangas in order to increase the accuracy and generalizability of the
findings. Second is conduction of follow-up and support trainings to further hone and
practice the skills of the BHWs. Finally is the provision of at least one IMC! manual that is
translated in the vernacular (Tagalog) for use by the BHWs in all barangay health centers
implementing the IMCI protocol.</subfield>
