   <subfield code="a">Urinary tract infection (UTI) is a common disease condition affecting all ages, particularly women. Laboratory diagnosis of UTI is complicated by the intake of antibiotics by the patients, this may lead to false negative culture results and the true etiologic agents are not identified.  The applicability of the BacT/Alert 3DTM blood culture system was evaluated for culture of urine samples to diagnose urinary tract infection through detection of bacteriuria. BacT/Alert PF bottles were utilized to determine the effect of its antibiotic neutralizing capability on urine samples of patients under antibiotic treatment.  Urine culture using the conventional method (Method A) was compared with the BacT/Alert 3DTM system (Method B).Adult walk-in patients of Hi-Precision Diagnostics, with or without antibiotic intake were recruited to participate in the study. A total of 306 urine specimens were processed both by direct Gram's stain and direct culture method using the calibrated loop method and Gram's stain and culture using Pediatric Fan (PF) bottles in Bact/Alert 3DTM system. Isolated organisms from both methods were matched. The number and percentage of positive and negative cultures were determined. The effect of the antimicrobial removing device of the PF bottle was analyzed; the specificity and sensitivity of the BacT/Alert  3DTM were computed; and the advantages and disadvantages were also determined.  Escherichia coli was the most frequently isolated organism among all the bacterial isolates and among the Gram negative organisms in both culture methods. The most typical isolates seen in the calibrated loop method were also seen in the Bact/Alert 3DTM Method. More types of organisms were isolated from  BacT/Alert 3DTM system than with the calibrated loop method, 17 and 11 types, respecively.  Out of 306 urine cultures, at 95% confidence interval, 79-88% (with an average of 84%) was observed to be positive in Method B as compared to the 11-20% positive (with an average of 15%) in Method A. Considering Method A as the standard, the calculated sensitivity and specificity of Method B was 100% and 18.85%, respectively. There were 149 patients with antibiotic intake, Method A, gave 31 (20.8%) positive urine cultures, Method B which has an antimicrobial removing device (ARD) showed higher positive culture results, 128 (85%).  The advantages of Method B are: higher sensitivity (detection of low bacteriuria); shorter detection time of positive cultures; more types of organisms recovered; neutralization of antibiotics and other inhibitory substances; automated monitoring and, therefore, less technologist time used for processing and interpretation of culture results. The disadvantages are the high cost of a PF bottle used for every urine sample and absence of an established interpretative guideline.  Thus, the  BacT/Alert 3DTM system evaluated in this study that has been originally used for blood samples is recommended as a screening method for culture of urine samples to diagnose urinary tract infections.</subfield>
